Phoenix PD officer shoots, kills man armed with knife at Phoenix mental health facility, officials say

No officers were injured in the shooting, the Phoenix Police Department said.

PHOENIX — The Phoenix Police Department has confirmed a suspect has died after a police shooting in west Phoenix Tuesday morning. 

The shooting occurred shortly after 10 a.m. at an inpatient mental health facility operating out of a home near 111th Avenue and Camelback Road, police said.

Police said officers were called to the home after receiving reports that a patient with a knife was making suicidal threats and threatening to hurt workers.

Authorities said officers contacted the man inside the two-story home and began to talk with him. Police said officers talked with the man for more than 15 minutes, trying to provide him help and offer assistance. 

Police said while they were talking with the man, he removed a knife from his pocket. Officers ordered the man to drop the knife, and he refused. Officials said an officer used a Taser to get the man to comply. The man showed no effects from the Taser and took a step toward the officers. One officer fired his gun, striking the man, police said.

Officials said the man was transported to the hospital in critical condition and later died from his injuries. 

No officers or civilians were injured, police said.

According to state records, Angel Heart Behavioral Health operates out of the home where the shooting occurred. 

The state shows this is a 24/7 facility with nine licensed beds. At least one staff member is supposed to supervise residents at all times.

12News found the facility has had several complaints. The Department of Health Services said those complaints were not substantiated, and this facility has had no citations.

12News made multiple attempts to contact the owner but hasn't been able to speak with him.  

“ADHS is aware of an incident reported today at this facility and will investigate from the perspective of licensing rules and regulations,” – a spokesperson for AZDHS said.

Police said the man's name is not being released pending notification of next of kin.

Calls to Silent Witness are answered 24/7 by a live person and submitted tips are accepted at all times. Submitted tips are then sent to the detective(s) in charge of the specific case.

Individuals who submit tips that lead to an arrest or indictment in the case can get a reward of up to $1,000.
